Student & Parent/Guardian SIGNATURES REQUIRED BELOW:

MN law requires a $100 down payment toward behind-the-wheel before a student is issued their digital "blue card" to take the permit test.

TOTAL amount due for digital blue card to take permit test: $100

It is hereby agreed to give 6 hours of Behind-The-Wheel instruction in relation to the operation of a motor vehicle for a fee of : $320.00

Minus down payment if paid toward the Behind-The-Wheel: --$100.00

TOTAL amount due before start of Behind-The-Wheel: $220.00

This agreement constitutes the entire contract between Mille-Lacs Driving Instruction and the student and no verbal statement or promises will be recognized. In the event of a cancellation, no-show, or no permit by a student of a scheduled BTW session, a $60 fee may be charged if a 48 hour advanced notice is not given.

Student also agrees to pay BTW fees. If not paid blue and white electronic cards will not be submitted to the state. If the student cannot complete the classroom or behind the wheel sessions a 50% refund will be issued.

Students may be required to have additional passengers in vehicle due to scheduling needs. Vehicle used for instruction and/or road test is equipped with automatic transmission, power steering, ABS, instructor brake and meets vehicle standards set by the Minnesota Department of Public Safety.

Note: MN drivers manuals are no longer free. Extra manuals will cost $5.00

Note: If fuel costs exceed $3.50 per gal a $5/session fuel surcharge may be added.

Note: If behind the wheel has not been completed in a year, the price will be the current amount.
